Per checking the details of the account, we noticed you have two-factor authentication enabled and per the logs, you have +90 trusted devices added, this might cause an issue for new devices or new browser tabs you want to save.
For security, we are unable to check these details. However, you may revoke trust for a device or delete a trusted device; please, check the article below:

This has been a problem on & off for me, and persistently at the moment.
I reviewed my trusted devices based on your comment - I have around 20, but all but two were the same browsers on the same computers (I have a code that I use to identify which one is which) that I had ticked the trust for 30 days box.
It appears that something somewhere is causing Lastpass to forget that device is trusted.
I think it many have something to do with changing geographic locations, as I travel and work from different locations, and my internet access, while from the same device (I only ever use my mobile data on when travelling, too many horror stories about using hotel and public wi-fi) will have a different server/access, not sure of the techincal terms here, somewhere in the network .
That might be only part of the possible cause - because today I have to not only login and autheticate, I also had to re-verify the location. Being the same location I worked from yesterday and my "home" work location.

Please see if this setting has reverted, and update it if necessary:
Access your Vault, click "Account Settings" > "Show Advanced Settings" > "Security" > "Website Auto log off and Bookmarklet Auto log off" :: set to 2 weeks, click update, enter you Master Password to save the changes.
