Per checking your description, if you are referring to the website icons in the vault, the default is that LastPass will display the icons if the website is recognized, based in the URL saved. If they display a "lock" icon, it is saved but not recognized.
However, there are some details to review. According to your logs, you are using the web (lastpass.com) and the Chrome extension. Can you review if there is a difference on the item if you log in differently?
If the issue persists, please, create a case with support to further review.

Karol...thanks for the reply. I understand your explanation + direction.
The very next day...all WEB ICONS (recognized) simply reappeared.
What remains as a mystery to me is why it disappears and a day or two later...reappears?
