So that I may check on the status of your account, please confirm: Are you referencing the IP/device verification emails?
In this case, LastPass will only trigger those if you need to re-confirm your IP after 60+ days, or are using a new device / IP address -AND- have entered the correct account password.
In both of your cases I do not see any unverified IPs or security requests over the last month of activity.
