I don't know how to describe it. In the picture, each line is supposed to a separate entry from the website. Instead, Lastpass is doing this

I would suggest some troubleshooting such as clearing the LastPass cache, and in a worst-case scenario try disabling specific autofill functions within your Vault's General settings.
