You can remove the LastPass app like you would any other application on your chosen device. Depending upon the set up and operating system, just make sure you also empty the 'trash' where files sometimes end up before a full reboot is performed.
With regards to deleting your entire account, first make sure you have cancelled any active LastPass subscriptions, and then follow these instructions.
