If you have automatic updates turned on by default, then you should now have version 4.151.5 for Safari.
Please let us know if you experience any further difficulty with this version, and specify any new behaviors identified.

I don't mean to sound snarky, but that doesn't sound like a comment from someone who has actually looked at the ratings and reviews on the App Store.
Can you point me to documentation showing that many of those issues have been addressed? Will LastPass offer me a refund (yes, I have a Premium subscription) if I have the same poor experience as so many others?

Thank you for taking the time to share your comments, I know that you might be hesitant to use the LastPass Extension on Safari, to be honest the transition should be seamless and you should be able to continue using and accesing your sites with no trouble. Here's a guide to get the Safari extension installed: Install and log in to LastPass for Safari on Mac.
Also your data should be save as it remains safe in your vault and you can access it as long as you have your Master password from our website or from the lastpass extension in other browsers.

The overall customer review in the app store may have several different reasons -- not just the app functionality.
Full transparency, there is an issue we're still tracking with Safari user profiles blocking some autofill actions, however this doesn't affect all users. Hopefully we'll be able to identify the blockage and see if an updated version can work around it.
Otherwise, feel free to test it out and let us know about any specific websites or browser app failures.
Do you have any updates to that issue on user profiles? It has been quite some time and looks like it is still around.
