How do I set up auto fill for a login dialogue generated by an app installed locally on my laptop (so there's no URL); it needs to insert a User Name & Password.

Welcome to the community!

Normally, when you are using LastPass for autofilling it always request you to add a URL to recognize or detect the item. There is no specific option for apps that are only apps and does not have a URL. I will suggest maybe trying with the custom item option.
