I don't have the answer but have experienced the exact same thing over the past few days. I cannot access my vault even though my UN and PW are accurate.
I also get this, no failed logins as far as I know, but it regularly displays this message, and kicks me out of the app.

Also, please confirm your General Settings online at lastpass.com for "Log out after this many minutes of inactivity" and "Log out when all browsers are closed".
