I will recommend you uninstall completely LastPass extension from your computer just put it in the trash and clear the trash for any LastPass app or log that you have inside the trash. Once you delete it, go to Safari > Settings or Preferences > Privacy > Manage Website Data > Delete LastPass cookies > Quit Safari. Then you can reinstall LastPass from the App Store in your Mac and re-enable the LastPass extension on Safari for use on all websites. Inside App Store you will find it as LastPass for Safari.
