Why is this icon showing up on my Google Form name?

I am COMPLETELY stumped. I've used Last Pass for years w/little problem other than it sometimes filling in rnadom fields incorrectly with emails and passwords ....but that's for another post.

My current issue is that I created a Google Form which I do frequently and when I open it there is a last pass icon in the corner of the title. I HAVE NO IDEA WHY but it's driving me crazy and I don't want the recipients of the form to be confused by it.

Any advice greatly appreciated!
