I hope this message finds you well. If this is only happening from the app my recommendation will be to verify that the app is up to date if it is tried uninstalling the app completely and reinstalling the app maybe there is something in the app not syncing properly with the LastPass server. Once done the process, keep an eye on the app for the next couple of days and if the issue persists let us know.
