What does "Save ALL Entered Data" mean? Is that an option somewhere? I don't see it.
Assuming unique fields, try capturing the entry again using Save All Entered Data.

I see the option now... to use + then all fields... however, after the first time login it does not fill in the frequent flyer number and my last name... can't figure out what is wrong. Looks good but does not work after initial login.
