I cannot generate One Time Passwords.

Question

I go to the screen and hit the button to do so, and it pops up a login dialog.I put in the password, and it just throws me back to the screen I came from with the button.I've removed and reinstalled the extension.  I cleared the browser cache.  Nothing is changing the behavior on this.  How do I get past this to create the OTPs?

If the issue persists, try to replicate the issue using other browser

I have done all those things in the guide as suggested.  Nothing has changed.  I don't know what is going on, but now LastPass is making me log in every time I open the browser instead of staying logged in as well.  And I still cannot generate one time passwords.  After I enter the master password, it just goes back to the previous screen.  I have tried multiple browsers.  All have the exact same behavior.
