Why do I see a lock icon for my stored passwords in my LastPass vault?

The answer provided by Support is of no help.I used to have most of my site logos but now, ALL of my sites show a generic lock icon :-(

It's just the default icon when there LastPass doesn't have a logo/favicon for that particular site.
