I just removed the old LastPass and installed the LastPass for Safari. I've checked to see that my extensions are up-to-date in Safari and Firefox. I'll let you know if the problem persists after this.

Can you please confirm what device and web browser you're using with LastPass?

Are you currently utilizing any other autofill app or native browser password saver?

Just to also inform you, we don't currently support Duck Duck Go due to some of its privacy settings, so you may see different behavior when utilizing LastPass in that environment.
