Perhaps you didn't test this, but without a "valid phone number" in that entry, your form won't submit. (Or, perhaps it does, but we can't see that). It does not matter that you put a perfectly valid phone number in the description. Your response is 5 months old, so apparently you haven't fixed this problem.

Some area codes are not recognized as valid to be able to submit the number in the Call Me request. As a workaround, you may submit the ticket and provide the phone number to be contacted in the description, our tech support will be able to proceed as needed.
