From your LastPass vault, go to the top right corner click on your email address
Click on Support center
Top right corner clicks on "Loging to Support"
Then Click on "Contact US"
You will be able to choose the best option for you, A callback request or a Support case.

In case you want to see your account history please access your account using a computer, in your vault please go to the bottom right corner find the menu "Advance settings" and find the account history.
