I reinstalled the extension to fix an autofill problem, and now the icon for LastPass will not appear in the Chrome toolbar at the top of the browser window. I have clicked enable on and off a couple times. Still no icon, so no easy way to open my vault.

I am sorry about the issues you are having with your extension. It seems you have not Pinned the extension. On the top right corner of your browser window, search for the extensions icon(It looks like a puzzle piece). By clicking on it you will see the list of extensions installed and enabled:
Then, to the right of the LastPass extension. Click on the icon that looks like a pin. Once you click it, the extension will show on the toolbar again:
