How can I save on Last Pass a 3-field requiring sign in form?

I have a login form that I want to save on Last Pass that has 3 fields to fill. Can I make it work with Last Pass? It has to do with some government service here in my country. Something like an IRS account. But their stupid website doesn't let me/asks me to save the password. Neither Chrome's autosave password asks neither Last Pass asks to save when I sign in. Is it some way to make it work?

For signing in, it requires: Username, Tax ID Number, and a Subscription Key (a stupid 30+ alphanumeric code that I can't change)

Thanks. I found how to add the 3-form sign in and keep it saved.

Will it disappear after the 30-days that the premium trial expires? Or will it work and after?

@peterwesterIt will remain, that method of adding sites is not dependent on the kind of plan you have.
