We can investigate further after some of this troubleshooting is complete.
After clearing the cache, you may need to log out, then back into LastPass to confirm your vault is decrypted again.
I will disable chrome and delete the cache, but can you explain where to find this: the cache clearing option is within the LastPass app menu itself.
Will you also tell me if this will restore my passwords to what they were or will this just prevent it going forward?

Can you please disable the Chrome autofill, clear the LastPass cache, and then test again by logging out / into the app again?
I am using a chrome password manager too. I clicked the link "maually cleard the lastpass cache." Under iOS it said:
I did #1, but Actions was not one of the choices. Please advise.

I'm sorry for that behavior, as it is definitely not normal.
Are you using any competing password managers, like your web browser's autofill functionality?
Have you manually cleared the LastPass cache through your LastPass app?
I still don't know what the Last Pass App is. I open last pass in my chrome browser. When I go to the app store I see Last Pass for Safari - is that the app I should download? But if you are telling me to go to last pass within the chrome browser, I would refer you again to the screenshot above - when I click on settings (in the darkened bottom left of the screenshot) I get the big window that is open and there is no Actions>Clear LastPass Cache.

Yes-- this is the app specific to Chrome or Safari, where you will see the LastPass icon in the upper-right of the browser window.
I finally figured it out thanks to Google's AI, I cleared the cache and my passwords seem to be restored.
